medium\\\">　　“恙虫咬人夺命”一时成为各地关注话题，“别来无恙”等成语被赋予了新的意思。而广州几位不幸\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">去世的老人生前曾经去过的晓港公园，则成为“禁地”，草坪被围起来，喷杀恙虫的农药也被紧急使用。\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">一场公共卫生危机，逐渐被消弭无形。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　就像人们对这件事情关心得快，忘记得也快一样，恙虫病从来就不是新发的疾病，也不是类似SA R S\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">那样的人群传染性疾病。从1952年开始，我国已经有恙虫疫情报告，到1985年之前，每年报告的发病数都\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">在1000例左右。此后一直到1990年，每年报告病例数都超过2000例。之后由于全国性的恙虫疫情报告取消\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">，再没有准确的数字。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　此次出现恙虫病病例的广州，其实是这种病的“老家”。1948年，正是在广州，中国学者首次分离到\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">恙虫病的病原体。1958年，广州地区经过努力后，基本控制了恙虫病的流行。但近年来，该病例有重新抬\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">头的趋势，几乎每年都有恙虫病的报告，但叮咬致死的案例并非每年都发生。据广州市疾控中心罗雷博士\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">介绍，广州市从1995年开始，对恙虫病又开始了系统的监测，其中近年来最集中的一次爆发在2006年。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　罗雷说，所谓的恙虫病，其实应该称作“丛林斑疹伤寒”，是通过恙虫传播的一类疾病。这类病既不\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">同于普通的病毒感染，也不是细菌感染，而是一种被称作“立克次体”的微生物感染后引发的身体病变。\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">而且引发恙虫病的，也不是传统的立克次体，而是一类后来被称作“恙虫病东方体”的病原体。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　恙虫是这类疾病的唯一传播媒介，但并非所有的恙虫都会传播恙虫病。据罗雷介绍，传播恙虫病的其\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">实是恙虫的幼虫，也就是恙螨。“恙螨之所以会传染恙虫病，原因就在于它在自然界的寄主携带恙虫病东\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">方体”。而这类病原体，在南方地区，多存在于鼠类身上，鼠类，恰恰又是恙螨主要的寄主。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　“由于第一代恙螨寄生在鼠类身上，产生的蛹里携带了恙虫病东方体，等待其成熟变成恙虫产卵后，\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">孵化出的第二代恙螨，这些恙螨如果继续寄生在鼠类身上，则没问题，如果叮咬了人，则会传播恙虫病”\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">。也因为如此，罗雷说，控制恙虫病的发生，关键就在于灭鼠和杀螨，同时在户外活动时，应注意个人防\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">护。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　恙虫病东方体之所以能致命，原因就在于“它是细胞内寄生的病原体”，南方医科大学公共卫生与热\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">带医学学院郑学礼说，这相当于人体内原有的抗体对其不能产生有效的作用，要产生细胞内抗体才行。虽\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">然使用抗生素能抑制病原体的危害，但“由于细胞膜的渗透性较差，不能及时产生效果”，所以基础免疫\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">能力较差的中老年人，有可能因为恙虫叮咬致命，而即便是免疫能力强的人，叮咬后也不免留下一个“焦\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">痂”之类的记号。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　“走南闯北”\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　在温暖潮湿的广州，各类虫子给居民带来的烦扰可谓不胜枚举。蚊、蠓、虱、蚤、蜱、螨等几乎都存\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">在于这片土地，其带来的皮肤瘙痒、过敏性皮炎、传染性疾病等时有发生。大多数时候，我们对这类生物\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">早已见怪不怪，只有当出现登革热、乙脑或者是少见的恙虫病、新型布尼亚和人粒细胞无形体病时，才会\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">猛然惊觉：原来这些小虫子如此可怕。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　其实对于这类生物传播疾病，在流行病学上，有一个更为概括的说法，即虫媒传染病。均是因为上述\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">的蚊、蠓、蜱、螨等“节肢动物”叮咬过后而感染发病的一类传染病。据郑学礼教授介绍，虫媒传染病最\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">大的特征是，媒介（即各类虫）以生物性传播或机械性传播的方式，将病原体从宿主动物传给健康动物或\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">人类，从而引起的一类传染性疾病。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　这类传染病其实非常常见。比如广州常见的伊蚊，就是登革热和黄热病的主要传播者，而按蚊，则是\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">疟疾何丝虫病的传递者。即便是恙虫病的学名中那个“斑疹伤寒”，其实还有虱、蚤等也能传播，只是其\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">临床特征有异于恙虫病而已。当然，这其中，最“厉害”的，仍属近年来“暴得大名”的蜱。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　在未因为叮咬人致死而“暴得大名”之前，蜱虫更多为东北林区的人们所认识。在那里，它被称作“\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">草爬子”，与它对应的疾病叫做“森林脑炎”。小时候在内蒙古牙克石市生活的邵先生对此记忆犹新，“\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">我们进山采蘑菇，都得全副武装，碰到草丛什么的都要小心翼翼，生怕被"草爬子"咬了。下山回家，得赶\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">紧换衣服，看身上有没有虫子，或者被咬的痕迹，如果有，就要赶紧去林区治疗森林脑炎的专科医院就诊\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">”。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan medium\\\">的动物身上寄生。曾在阿坝州林业部门工作多年的张先生说，“我们进山，一怕蚂蝗，二怕"牛虱子"。"\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">牛虱子"咬了人过后虽然不会生病，但很难拔下来，很讨厌”。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　在东北和四川，蜱虫得到的是截然不同的评价。原因就在于虽然他们都姓“蜱”，但“蜱性”却并不\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">完全一致。东北的蜱虫多是“全沟硬蜱”，它们正是森林脑炎的主要传播者，也是相当长一段时间内，国\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">内防控蜱虫的主要对象，直到河南出现蜱虫携带新型布尼亚病毒。而四川的蜱虫多是“龟形花蜱”之类的\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">南方种，除了“嗜血成癖”以外，倒也不易出现大害。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　实际上，各类具有叮咬特征的节肢动物会否对人体健康产生危害，其主要原因正是取决于其生长的环\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">境。如果其所处的地区，恰好有某一类病原体的分布，这一地区就被称作“自然疫源地”，这些地区生活\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">的这类虫子，就是潜在的病源传播者；而如果这一地区没有该类病原体的分布，那这些虫子的存在，顶多\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">也就是“讨厌”一点而已，不至于危及生命。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　遗憾的是，同属于分类学上“蛛形纲”的蜱虫和恙虫，现在几乎已经在全国都有分布。蜱虫更有多达\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">100多个品种，其中大部分均能传播各类疾病，比如森林脑炎、出血热、回归热之类的传染性疾病。而恙\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">虫，则也早已跨过上世纪80年代之前的北纬31°线（之前主要在南方地区有恙虫病），据罗雷介绍，现在\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">在长江以北的多个省份均出现过恙虫病病例。“蛛形纲”里的各类螨虫，更是多种皮肤过敏乃至部分呼吸\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">道感染疾病的罪魁祸首。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　而对于南方地区，登革热、乙脑等虫媒传染病，则将是持续的防控重点，郑学礼说，虽然大规模爆发\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">的可能性不大，但彻底消灭的可能性也很小。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　无法消灭\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　和进入东北林区，知道它是森林脑炎的“自然疫源地”，可进行针对性的防护不一样，局部爆发的恙\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">虫病、登革热、乙脑等疾病，几乎没有确定的“疫源地”，而一旦流行，也只能对这些地点进行治理，对\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">外围进行防控。但你依旧无法预测，在什么时候、什么地点，可能碰上这类轻则使人看似“感冒发烧”，\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">重则让人多器官衰竭而亡的虫子。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　这些疾病不能彻底消失的原因，首先是自然疫源地的存在。我们的防控工作，则多是集中在对媒介生\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">物的防控上，而一旦这类媒介生物的数量反弹，疾病再次流行的可能性就迅速增加。尤其是多种虫媒传染\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">病的动物寄主是极其难消灭的鼠类。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　广州市晓港公园的恙虫叮咬事件发生后，专家们一致认为，主要是因为这里的恙虫寄生过的鼠类携带\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">恙虫病东方体。但这些鼠类从何而来，为何造成局地性的恙虫爆发，则不得而知。而防控措施，也是临时\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">加大灭鼠力度，同时在公园内消杀恙螨。但和老鼠接触比较多的猫，也可能成为传播的途径。罗雷等在晓\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">港公园的调查就发现，该地平时有很多猫活动。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　从上世纪全民“除四害”开始，我们与老鼠、蚊子、蟑螂等“亲近”人类的动物之间几乎从未停止过\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">“战争”，但结果往往是“此消彼长”，甚至提高了这类动物的生存能力。以鼠类为例，广州市近年曾多\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">次爆出鼠患的消息，比如杨箕村等城中村改造时，就曾造成鼠类临时“大迁移”，在多个餐饮业集中的地\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">段，老鼠更是大摇大摆的出入于下水道通道等场所。而它们是否携带病毒，或者会否通过蚊子、恙虫等媒\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">介生物传播给人类，没有确切答案。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　而这一切的变化，最终仍要归因于环境的变迁。郑学礼认为，广州近年多次发生登革热疫情，就与城\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">市环境的改变有一定的关系，“环境影响蚊虫感受态主要涉及温度、幼虫营养、病毒感染剂量等”，“广\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">州市登革热疫情的发生，除了流动人口增加以外，是否与广州的城市化建设，城中村等蚊虫孳生环境改变\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">，引起白纹伊蚊对登革病毒感受态变化有关，值得研究。”\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　而另一个值得担心的因素则是气候变化，以及随之而来的极端天气增加。郑学礼说，已有多项研究证\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">明，由于气温升高，全球不少地区已经出现虫媒传染疾病流行范围增大的案例。在中国，也出现了恙虫病\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">“北移”的现象。而局地干旱等极端气候，更可能造成昆虫的大爆发，继而引发相关虫媒传染病的爆发。\u003C/span>\u003C/p>\\r\\n\u003Cp>\u003Cspan style=\\\"font-size: medium\\\">　　对于各类虫媒传染病的防控，罗雷介绍有三个方面，即环境防治、化学防治和个人防护。在我们不知\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">情的情况下，“最好的防护就是，在户外，不要让虫子接触到你的身体”。虽然“在及时发现和救治的情\u003C/span>\u003Cspan style=\\\"font-size: medium\\\">况下，恙虫病等虫媒传染病的病死率在1%以下”。\u003Cbr
